FOLLOW-UP AT HOME
Optimizing the process of follow-up home consultations after hospitalization can be highly beneficial for patients returning to their home countries. Ratchaphruek Hospital aims to enhance their services by providing comprehensive treatment, patient care, and easy access to up-to-date health information, preventive measures, diagnostic services, and health education. This extension of services ensures a seamless continuum of care for patients once they are discharged.
● An International Medical Coordinator will be assigned to monitor the patient’s ongoing condition, assess any side effects from home medications, and identify any potential abnormal treatment outcomes.
● Patients are advised to maintain regular contact with their healthcare professionals for any developments, potential signs of infection, or pain related to their surgeries in order to prevent further complications.
● Following discharge, the responsible healthcare professional or medical inquiry team will reach out to the patient through international phone calls, telemedicine platforms such as Zoom, or even through messaging applications like WhatsApp. They will make at least two contacts: the first on Day 1 and the second on Day 7.
